01. Why look beyond body weight?
RETROD is our name for retatrutide research material. The research story reaches beyond a number on the scale: investigators have measured fat inside the liver, blood glucose, body composition, appetite, knee pain and physical function.
This guide brings those results together by outcome, with the study population and measurement period beside the numbers. The amounts shown in tables identify trial groups, not recommended personal doses. Clinical findings describe the study formulations; they are not test results for an MGMT vial.
02. Three receptors, one research molecule
Retatrutide activates GIP, GLP-1 and glucagon receptors. This combination joins pathways involved in appetite and glucose regulation with glucagon-related energy metabolism. In preclinical experiments, glucagon-receptor activity contributed to increased energy expenditure alongside reduced food intake. [1]
That mechanism helps explain the interest in outcomes beyond weight loss. It does not mean that a sensation of warmth proves increased metabolism, or that an animal energy-expenditure result provides a quantified human effect.
An early human pharmacokinetic study reported a half-life of approximately six days, supporting investigation of once-weekly administration. [2]
03. Liver fat: the complete dose-group results
The phase 2a MASLD substudy included 98 participants with at least 10% liver fat at baseline. MRI-PDFF measured the following changes. [3]
| Study group | Week 24 | Week 48 | Liver fat below 5% at week 24 |
|---|---|---|---|
| Placebo | +0.3% | −4.6% | 0% |
| 1 mg | −42.9% | −51.3% | 27% |
| 4 mg | −57.0% | −59.0% | 52% |
| 8 mg | −81.4% | −81.7% | 79% |
| 12 mg | −82.4% | −86.0% | 86% |
These are relative liver-fat changes, not total weight loss or proof of fibrosis reversal.
04. Liver biomarkers, triglycerides and ketones
Triglycerides improved versus placebo at ≥4 mg. K-18 and Pro-C3 decreased in selected groups; ALT, AST, FIB-4 and ELF showed no consistent improvement. [3]
β-hydroxybutyrate increased roughly two- to threefold at ≥4 mg, supporting fatty-acid metabolism research, without quantifying an individual’s fat-burning rate.
05. Body composition: fat mass and lean mass
A DXA substudy enrolled 189 adults with type 2 diabetes. Its primary assessment was change in total fat mass at 36 weeks. [4]
| Study group | Fat-mass reduction |
|---|---|
| Retatrutide 0.5 mg | 4.9% |
| Retatrutide 4 mg, pooled | 15.2% |
| Retatrutide 8 mg, pooled | 26.1% |
| Retatrutide 12 mg | 23.2% |
| Dulaglutide 1.5 mg | 2.6% |
| Placebo | 4.5% |
Lean mass also decreased. The authors found that its contribution to overall weight loss was similar to that seen with other obesity treatments. These results support substantial fat loss, not a promise of muscle preservation or muscle gain.
DXA separates tissue compartments more usefully than a bathroom scale alone. “Lean mass” includes more than skeletal muscle, so it should not be treated as a direct measure of strength.
06. Blood glucose: phase 2 and phase 3 findings
In the phase 2 type 2 diabetes trial, HbA1c changes at week 24 differed by dose and escalation group. Values below are percentage-point reductions, not relative percentages. [5]
| Study group | HbA1c reduction |
|---|---|
| Retatrutide 0.5 mg | 0.43 points |
| Retatrutide 4 mg, with escalation | 1.39 points |
| Retatrutide 4 mg, without escalation | 1.30 points |
| Retatrutide 8 mg, slower escalation | 1.99 points |
| Retatrutide 8 mg, faster escalation | 1.88 points |
| Retatrutide 12 mg, with escalation | 2.02 points |
| Dulaglutide 1.5 mg | 1.41 points |
| Placebo | 0.01 points |
Lilly subsequently reported TRANSCEND-T2D-1 results in 537 adults with type 2 diabetes. Mean starting HbA1c was 7.9%. The following 40-week figures use the efficacy estimand. [6]
| Group | HbA1c reduction | Body-weight reduction |
|---|---|---|
| 4 mg | 1.7 points | 11.5% |
| 9 mg | 2.0 points | 15.5% |
| 12 mg | 1.9 points | 16.8% |
| Placebo | 0.8 points | 2.5% |
Results in a diabetes population answer a different question from results in an obesity trial without diabetes. Neither table establishes diabetes remission or justifies changing prescribed treatment.
07. Knee pain and easier movement
TRIUMPH-4 studied 445 adults with obesity or overweight and knee osteoarthritis over 68 weeks. Lilly reported the following efficacy-estimand results. [7]
| Outcome | 9 mg | 12 mg | Placebo |
|---|---|---|---|
| Body-weight reduction | 26.4% | 28.7% | 2.1% |
| WOMAC pain improvement | 4.5 points | 4.4 points | 2.4 points |
| WOMAC function improvement | 4.1 points | 4.2 points | 1.7 points |
Average baseline pain was 6.0. These results concern knee osteoarthritis, not demonstrated relief of back pain or repair of cartilage. The illustration above represents a positive conversation, not a clinical outcome.
08. Cardiometabolic markers and the wider research programme
TRANSCEND-T2D-1 also reported improvements in triglycerides, non-HDL cholesterol and systolic blood pressure. These markers help describe metabolic health alongside glucose and weight. [6]
The development programme includes investigations in obesity, type 2 diabetes, knee osteoarthritis, obstructive sleep apnoea, chronic low back pain, cardiovascular and kidney outcomes, and MASLD. A condition being studied is not the same as an established treatment benefit.
A lower risk-marker reading and fewer clinical events are different outcomes. Claims about preventing heart attacks, strokes or kidney failure require trials measuring those events.
09. Appetite, hunger and “food noise”
There is retatrutide-specific research on eating behaviour. An exploratory analysis of 275 adults with type 2 diabetes assessed appetite questionnaires at 24 and 36 weeks. [8]
At week 24, groups receiving at least 4 mg reported greater reductions in appetite, hunger and anticipated food consumption than placebo. The 8 mg and 12 mg groups also showed improvements in perceived hunger and tendency to overeat at both assessments. Greater dietary restraint versus placebo was seen with 12 mg at week 36.
Changes in these measures correlated with weight reduction. “Food noise” is an informal phrase; the study used defined questionnaires rather than that single label. It does not establish that retatrutide treats alcohol dependence, compulsive behaviour or every kind of craving.
10. The weight-loss context and why time matters
The phase 2 obesity trial enrolled 338 adults. Its mean body-weight reductions provide context for the other outcomes in this guide. [9]
| Group | Week 24 | Week 48 |
|---|---|---|
| 1 mg | 7.2% | 8.7% |
| 4 mg | 12.9% | 17.1% |
| 8 mg | 17.3% | 22.8% |
| 12 mg | 17.5% | 24.2% |
| Placebo | 1.6% | 2.1% |
These are group averages assessed over months. A trial’s highest dose is not automatically the right dose for an individual. Comparing results across different trials also requires attention to participants, duration, treatment adherence and analysis methods.

11. Tolerability: the actual reported percentages
TRIUMPH-4 reported these adverse-event rates. “Dysesthesia” means altered or unpleasant skin sensation. [7]
| Event | 9 mg | 12 mg | Placebo |
|---|---|---|---|
| Nausea | 38.1% | 43.2% | 10.7% |
| Diarrhoea | 34.7% | 33.1% | 13.4% |
| Constipation | 21.8% | 25.0% | 8.7% |
| Vomiting | 20.4% | 20.9% | 0% |
| Decreased appetite | 19.0% | 18.2% | 9.4% |
| Dysesthesia | 8.8% | 20.9% | 0.7% |
| Discontinued because of adverse events | 12.2% | 18.2% | 4.0% |
In the phase 2 obesity trial, gastrointestinal events were generally mild to moderate and dose-related. A lower starting dose reduced their frequency; dose-related heart-rate increases peaked at week 24 and subsequently declined. [9]
These tables are not a complete safety profile. A reported side effect should not be treated as evidence that a product is working or that a vial has the expected identity or purity.
12. Reading results, labels and batch reports
Keep three kinds of information separate: the molecule’s clinical evidence, a research product’s labelled quantity, and the analytical findings for a specific batch. A 20 mg vial label describes the vial’s contents; it does not mean a 20 mg dose.
A COA provides the results of the tests listed on that report. It does not transfer a clinical trial’s results to a commercial vial or establish suitability for human use. Review the matching lot on COA & Batch Reports, including whether its report is available or pending.
For storage and laboratory handling, follow the product-specific documentation. For personal symptoms, medication changes or health monitoring, work with a qualified clinician.
13. Questions that come up often
Does less liver fat mean liver disease is cured?
No. Fat content, inflammation and fibrosis are different endpoints. An imaging result alone does not answer every liver-health question.
Does losing weight mean all the loss is fat?
No. Body composition measures help distinguish fat and lean tissue. The DXA study above reported changes in both.
Do warmer skin or more energy prove increased fat burning?
No. Subjective feelings cannot quantify energy expenditure or verify a product.
Can retatrutide be ranked against other compounds using separate trial percentages?
Separate trials provide context, but a numerical ranking is not a controlled head-to-head comparison. Study design and population matter.
Are these customer results?
No. This page contains study findings and research explanations, not customer stories or testimonials.
